Prior to your initial visit, you may print the below listed forms to acquaint yourself with the policies and procedures necessary to operate a good dental practice.

Some of the forms are now required by HIPPA (Health Insurance Portability and Privacy Act) implemented in 2003. Other forms relating personal, medical and dental history are important as well. These forms  help us get acquainted and better understand you and your dental needs.
